A connected care pathway
From scheduled visits and daily personal assistance to bedside procedures and prescribed therapies, each service can form part of a coordinated plan.
01
Scheduled nurse visits by SANC-registered Professional Nurses (PNs) and Staff Nurses (RSNs), assistance with Activities of Daily Living (ADLs: hygiene, bathing, dressing, mobility), and vital signs tracking (blood pressure, blood glucose, Hb, oxygen, fluid balance).
02
24-hour long-term clinical oversight for frail older adults, short-term caregiver respite, post-hospital step-down recovery, and GP-guided palliative/end-of-life support.
Post-hospital step-down recovery: Pending Facility Registration
03
Urinary catheter management (insertion, removal, periodic changing, urine chemistry), specimen collection for PathCare labs, ear syringing, and enema/suppository administration.
04
Advanced dressings for surgical, vascular, pressure, or diabetic wounds, high-definition digital photography tracking, care plans, direct GP reporting, and suture/staple removal.
05
Medication compliance support, blister-pack coordination, prescribed adult injections (subcutaneous and intramuscular), and oxygen/IV therapy executed under direct GP prescription.
Care at home
The everyday and the clinical belong in one clear picture. Our team observes, assists, records and communicates so patients and families are not left to manage care alone.
Daily living and mobility
Respectful help with hygiene, bathing, dressing and movement, delivered in a way that supports comfort and independence.
Observation and tracking
Vital signs, fluid balance and wound progress are documented to provide a clearer view of ongoing care needs.
Family and caregiver relief
Scheduled support and short-term respite give caregivers practical assistance while maintaining continuity for the person in their care.
Professional coordination
Where treatment requires a prescription or clinical direction, care is coordinated with the patient’s GP and carried out according to the authorised plan.
SANC-registered nursing professionals
Direct GP reporting for specialised wound care
Prescribed injections, oxygen and IV therapy
Specimen collection for PathCare laboratories
